IV Nutrient Infusions and Hydration Vitamin Injections and Peptides Medically Supervised Weight Loss Programs Clinical Nutritional Counseling All services are provided following a medical evaluation and depend on client health eligibility.

  1. MEDICAL ELIGIBILITY & HEALTH DISCLOSURE You represent that all health information provided is accurate and complete. You agree to disclose all known allergies, medical conditions, and medications currently being taken. We reserve the right to refuse service based on clinical judgment or safety concerns.

  2. PAYMENTS, CANCELLATIONS & FEES Payment for services reflects the appointment time and medical supplies used. Full payment is required at the time services are rendered. Cancellations within 24 hours of a scheduled appointment may be subject to a cancellation fee as defined in our clinical policy. Refunds for completed services are not provided under any circumstances due to the medical nature of the supplies and professional time utilized.

LIMITATION OF LIABILITY IV Wellness Solution and its medical staff shall not be liable for any indirect, incidental, special, or consequential damages resulting from your use of the website or clinical services. While we adhere to high clinical standards, results from wellness treatments may vary based on individual physiological factors.
